Information for non-specialist clinicians on prescribing Home Oxygen:
From the 26th March a new Home Oxygen Order Form (HOOF) will need to be used for patients who require oxygen for palliative care, cluster headaches or prior to formal assessment by your local Home Oxygen Assessment and Review (HOS-AR) team. This level of prescribing is known at Part A and it is only this section of the HOOF that GPs will need to prescribe a temporary supply of oxygen.
Please make sure you familiarise yourself on how to complete the new Part A HOOF so that this is not a problem when you come to need it. You can access instructions at the following website. There is a guide that you can print off and keep handy and you can also watch a very quick video showing the form being completed.

In the unusual event that your patient requires ambulatory Oxygen please contact your local HOS-AR Service who will be able to offer advice as to next steps. For patients who require a full oxygen assessment please refer the patient to the Community Respiratory Service.
