Local administrative information

We are a team of nurses and physios experienced in the care of the elderly with chronic conditions. We also have expertise in supporting End of Life care and have a specialist nurse who helps support the Homes with Advanced Care Planning etc. We also have access to a Mental Health Liaison nurse and a Speech and Language therapist.

Inclusions:
The only referral criteria is that the person is resident in a Care Home with Nursing in Brighton and Hove.

Referral is by phone: 01273 265610 or fax 01273 693081. Our working hours are Monday – Friday 0830 – 1630 and excludes week-ends and bank holidays
