The Chronic Respiratory Diseases Service provides an integrated city wide service for people with Chronic Obstructive Pulmonary
Disease (COPD), working alongside existing and developing services within the community, providing care closer to people's homes.
The service aims to enable people living with COPD to better manage their condition, preventing hospital admission and facilitation
of hospital discharge.
Inclusions
- Diagnosis COPD
- Over 18 years old
- Registered with a Brighton & Hove GP
