Referrals to community matrons

This section is provided for health professionals (GPs, dentists, pharmacists etc).

Members of the general public are welcome to use these pages but should be aware that they are not written with them in mind. Please choose an option on the left for information aimed at the general public.

Inclusions

Individuals aged 65 or over: (below age 65 will be reviewed for appropriateness)

Referrals are accepted from the 17 GP practices linked to the service. In addition the Community Matrons use a case finding tool to
identify VHIU patients from hospital admission data.
