District nurses - day
This service provides holistic and effective nursing care including health promotion and health screening to housebound adults or to those who by reason of their health needs, are best supported in their own environment by the skills and expertise of the DN service.

District nurses - night
The Out of Hours District Nursing Service provides skilled nursing care for people who are housebound from 19.45hours to 08.00
hours 365 days a year (excluding nursing homes).
It provides nursing care to adults over the age of 16 years who have an urgent nursing need that cannot wait until the next day.
It provides the following core services:
- End of Life care
- Catheter care
- Wound assessment and treatment
- Medication injections including Intravenous antibiotics
- Verification of death and support to families
- Assistance to patients to enable them to stay at home and reduce hospital admissions
- A night sitting service is also available.
Urgent calls will receive a visit within 30mins. The service is developing a night-sitting service for patients requiring extra support through the night, provided by Health Care Assistants.

GPs refer by telephoning either the team direct 01273 667303 or by using the call centre number 0845 434 9655.
